In-Vitro Fertilization or IVF In-Vitro Fertilization or IVF is an Assisted Reproduction Technology or ART used to help treat patients with Infertility and assist with the conception of a baby. IVF process involves the collection or extraction of mature eggs from ovaries and fertilized by sperm in a lab. The fertilized egg (embryo) or eggs (embryos) are then transferred to the uterus.
The Necessity of IVF Treatment
IVF treatment is done or performed to treat infertility after the couple already had tried less invasive treatment options that include taking fertility drugs to increase the production of eggs, or Intrauterine insemination – IUI. Sometimes, the IVF treatment may be required for infertility in women above the age of 40. IVF can be done to treat infertility in the following circumstances
Fallopian tube damage or blockage.
Previous tubal sterilization or removal
Impaired sperm production
Fertility preservation for cancer or other health conditions
Preparation for IVF at KIC – Chennai – An overview
Before beginning a cycle of IVF using eggs and sperm, the couple are subjected to various screening in the form of tests that include-
Ovarian reserve testing
Ovarian reserve testing is done to determine the quantity and quality of the eggs, by testing the concentration of follicle-stimulating hormone (FSH), estradiol (estrogen) and Anti-mullerian hormone in your blood during the first few days of your menstrual cycle. Test results often used together with an ultrasound of the ovaries that can help predict how the ovaries are responding to fertility medication.
Semen analysis is done as part of an initial fertility evaluation where the doctor will conduct a semen analysis shortly before the start of an IVF treatment cycle.
Infectious disease screening
The couple will both be screened for infectious diseases, including for HIV.
Practice (mock) embryo transfer
The infertility specialist may conduct a mock embryo transfer to determine the depth of the uterine cavity and the technique most likely to successfully place the embryos into the uterus.
The doctor will examine the inside lining of the uterus before starting the procedure of IVF. This might involve a sonohysterography — in which fluid is injected through the cervix into the uterus and an ultrasound to create images of the uterine cavity or it might include a hysteroscopy in which a thin, flexible, lighted telescope (hysteroscopy) is inserted through your vagina and cervix into the uterus.
What to expect during IVF?
IVF involves several steps that include ovarian stimulation, egg retrieval, sperm retrieval, fertilization and embryo transfer. Usually, one cycle of IVF can take about two to three weeks and more than one cycle may be required in some cases.
Ovulation induction – stimulation of egg growth
To increase the chances of pregnancy, the ovaries are stimulated to stimulate more than one egg to develop at a time through fertility drugs /RFSH/ HMG. Medications are also used for Oocyte maturation such as Human Chorionic Gonadotropin (HCG) to help eggs mature. On the day of egg retrieval, the doctor might also recommend taking progesterone supplements to make the lining of the uterus more receptive to implantation or might opt for frozen embryo transfer.
Egg retrieval is done in the KIC – Chennai centre in about 34 to 36 hours after the final injection and before ovulation. Mature eggs are collected in a pteri dish by placing them in a nutritive media. Healthy eggs and those matured are mixed with sperm for the creation of embryos. However, not all eggs may be successfully fertilized.
Sperm is taken by masturbation or through testicular aspiration /Testicular biopsy in the case of Azoospermia and undergoes a series of lab procedures to prepare it for interaction with the eggs.
Embryo transfer is done at KIC – Chennai Centre and usually takes place two/ three or five days after egg retrieval. The embryo transfer procedure is usually painless and done by placing the embryo or embryos into the uterus with the help of a small plastic catheter. The patient can return home after some rest and if successful the embryo will implant in the lining of the uterus in six to ten days after egg retrieval.
Medications are usually prescribed to support the young embryos hormonally. The presence of pregnancy is established after 12 days or two weeks after egg retrieval by using a sensitive test for Human Chorionic Gonadotropin (HCG) – a hormone secreted by the placenta. In case, the pregnancy is established which a very likely then monitoring is continued until the development of the fetus on day 28 following embryo transfer.
